Bank Account Form
Opening bank account: For attestation of forms required for opening a bank account, the applicant has to appear in person at the Consulate General with his/her Valid Bangladesh passport.
Documents required at the time of submission of application are:
- Filled in Application Form for attestation/certification;
- Prescribed Fees per document {US $ 44.00 for normal delivery on the next working day and US $ 82.50 for urgent delivery on the same day between 03:00-05:00 p.m.} payable through a bank draft/certified or official check /money order in the name of the Consulate General of Bangladesh in New York;
- Filled in account opening form (to be obtained from the bank);
- Specimen Signature (SS) card (to be obtained from the bank);
- Photocopy of account opening form and SS card;
- Three copies of photograph with name printed on the back; and
- Photocopies of Bangladesh passport of the applicant.
1. Requirements for Attestation of Other Documents (Affidavit/Bank Form/Mutation related papers, etc.)(Appointment)::
01. Original documents must be notarized by a Notary Public
02. A duly filled in prescribed application form for attestation.
03. Applicant(s) is (are) required to appear before the Consular Officer for thumb impression and signature in his/her photographs.
04. Photocopy (ies) of Bangladesh Passport or, Dual Nationality Certificate or, National Identity Card of Bangladesh of the Applicant(s)/Principal(s)/Executant (s)/Attorney(ies).
05. Recent Passport size colour photographs on a white background 02 (two) copies.
06. Photocopy of any US Photo ID.
07. Photocopy of court order in case of name change.
08. A set of photocopies along with the original document/original power of attorney.
09. The following applicable fees through Money order/Bank Draft or Bank Certified Cheque:
Urgent fee USD 82.50: Same working day.
Regular fee USD 44.00: Next working day. A Paid Trackable Return Envelope.
6. Requirements for Attestation of Other Documents(Affidavit/BankForm/Mutation related papers, etc.)(Appointment)::
In case of US Citizen or, if executants(s)/Principal(s)/Signatory(ies)
does (do) not have Bangladesh passport/Dual Nationality Certificate/National Identity Card of Bangladesh, the original document is required authentication step by step from the following offices and followed by the attestation from the Consulate General of Bangladesh in New York.
01. Step 1st Original documents must be notarized by a Notary Public
Step 2nd Authentication of Commission of the Notary Public by the City Clerk or County Clerk.
